- Physician certification completed 120 days or less prior to moving in.
- Physical and cognitive assessment performed by one of our professional nurses.

At Terra Vista, our residents are not defined solely by their respective diagnoses. We work with residents and their families to learn their stories and customize care for the individual. We request that prior to moving in, families answer life story questions regarding the resident’s daily life, their routines, interests, values, likes, dislikes, and other valuable information that provides insight to the individual.
When it’s time for your loved one to move to Terra Vista of Oakbrook Terrace, our Family Relations Director will be with your family every step of the way to assist in a smooth transition and acclimation period.
