Transitioning To Memory Care

The memory care experts at our dementia care community in Oakbrook Terrace, Illinois have made the process as simple as possible so the focus is on the wellbeing of your loved one during the process of dementia transitioning. To make the transition into our memory care community more smooth, we’ve broken the process down into 3 steps:

  • Physician certification completed 120 days or less prior to moving in.
  • Physical and cognitive assessment performed by one of the professional nurses at our dementia care community
  • Medical history and physical health information gathered by our staff.

At Terra Vista, our residents are not defined solely by their respective diagnoses. We work with residents and their families to learn their stories and customize care for the individual. We request that prior to moving in, families answer life story questions regarding the resident’s daily life, their routines, interests, values, likes, dislikes, and other valuable information that provides insight to the individual.
